The pancreas is a retroperitoneal organ.
That means that it lies just beind the peritoneum, which is behind a lot of other organs.
So, in acute pancreatitis, all the organs were initially resting on the pancreas, now due their pressure, will compress it . Therefore, there would be pain.
The ones of which I am unsure about
:
There is a coeliac plexus behind the pancreas. It gets damaged during the process of pancreatitis. So if the pancreas presses on it, it causes irritation and so the pain.
There was also something about fluid draining into the paracolic gutters when the patient leans forward.
