You basically get a feeling for it. When the engine becomes noisy, you shift up, and when the engine begins to run unsmooth, because it revs too low, then you shift down. It is more or less about keeping the revs of the engine in that area where the engine runs smoothly and delivers enough torque. 

Depending on the engine it is basically a range between 2,000 and 5,000 rpm. A rev meter may help you to learn how the engine feels and sounds at certain revs, but as soon as you are used to manual shifting, you won't need it.

Another factor is speed. You only use 1st gear for driving really slow (parking, starting from zero). 2nd gear is good as soon as the car is rolling. 3rd gear is good for driving in the city. 4th gear is good for driving in the city on stretched, wide streets with continuous traffic or on curvy roads outside city limits, and 5th gear is good for driving constant, high speeds i.e. on highways.
