the image will be formed at D.
reason:for the formation of image,atleast two rays are required.here,first ray will be perpendicular from object to the mirror and the second ray is found by producing backward, the reflected ray.

three is the answer to first question
the number of images are found by formula :360/angle -1
so,360/90-1 = 4-1= 3
and the third question is not complete so I cannot help in that
